Deaconess Memorial Medical Center invites public input for Community Health Needs Survey

JASPER Deaconess Memorial Medical Center is inviting residents across the region to share their perspectives on local wellness priorities by participating in its Community Health Needs Assessment (CHNA) survey.

The survey is currently open and will remain available through Sunday, July 26, 2026. Hospital officials will use the collected data to evaluate regional health challenges, identify service gaps, and guide future community benefit programs and healthcare services.

Shaping the Future of Local Healthcare

The Community Health Needs Assessment is conducted every three years in accordance with federal requirements under the Affordable Care Act. The feedback collected directly influences how the hospital allocates resources, targets preventive health programs, and collaborates with county health departments and community organizations across Dubois, Martin, Pike, and Spencer counties.

Prior assessments have helped pinpoint key regional health priorities, including expanded access to behavioral and mental healthcare, improved chronic disease management, substance use recovery resources, and enhanced access to local primary care services.

Participation details:

  • Time Required: Approximately 10 minutes to complete.
  • Privacy: All responses are completely anonymous.
  • Eligibility: Open to all community members, residents, and local stakeholders living or working in the service area.
