Jeffrey Scott Collins of Mitchell passed away Tuesday afternoon at Methodist Hospital in Indianapolis at the age of 41.
Born in 1967 in Leesburg, Florida to Bill and Judith Collins, Jeffrey was a former employee at Tieman Tire.
He is survived by his mother, wife Lavonda, daughter Kayla, brothers Billy, Mitch and Mike, and sister Donna.
He is preceded in death by his father.
Graveside services will be held Friday morning at Mitchell Cemetery with burial to follow.
No visitation has been scheduled.
