Phyllis C. Jacobs, 75, Mitchell

Phyllis C. Jacobs of Mitchell passed away late Saturday morning at Methodist Hospital in Indianapolis at the age of 75.

Visitation will be Thursday evening from 4:00 until 7:00PM at Chastain Funeral Home.
