FSSA oversees Level of Care Assessments for aging, wellness, and brain injury programs 

 INDIANA – The Family and Social Services Administration monitors and oversees the administration of federally required Level of Care assessments and determinations, as well as intake counseling for individuals in need of Nursing Facility Level of Care Assessments. Nursing Facility Level of Care assessments are completed for the Health & Wellness, PathWays for Aging, and Traumatic Brain Injury waivers, as well as nursing facility admissions. 

FSSA is dedicated to enhancing the quality and efficiency of evaluating functional eligibility to access services and supports. The Level of Care Assessment Representative is a process designed to streamline the functional eligibility evaluation to determine the Nursing Facility Level of Care. 

Beginning July 1, 2025, the LCAR will provide:

  • Preadmission Screening and Resident Review 
  • LOC assessments and renewals
  • LOC determination recommendations and re-determinations
  • Intake counseling for the Indiana PathWays for Aging 60+ population
