INDIANA – Ascension, a major healthcare provider, announced on Monday a “privacy incident” that compromised patient information across several states, including Indiana. The breach also affected individuals in Alabama, Michigan, Tennessee, and Texas.

The incident was discovered on December 5 when Ascension became aware of a potential security breach involving patient data. An internal investigation revealed that patient information was inadvertently disclosed to a former business partner and subsequently likely stolen due to a vulnerability in third-party software utilized by that partner.
The compromised data includes a range of sensitive information, such as names, addresses, phone numbers, email addresses, dates of birth, race, gender, and Social Security numbers. Clinical information related to inpatient visits was also affected.
Ascension is offering affected individuals two years of complimentary credit monitoring and identity theft protection services through Kroll. The health care provider has established a dedicated call center for inquiries, reachable at 866-408-3556 during weekdays.
In Indiana, Ascension operates facilities in various locations, including the Indianapolis area, Anderson, Boonville, Evansville, Lafayette, Newburgh, and North Vernon.
