According to an Indiana State Police report, 56-year-old Mark Horn of Chrisney was operating an ATV that left the roadway for an unknown reason, causing him to be ejected off the vehicle. The ATV came back onto the roadway and rolled onto its side.

Residents near the accident scene called 911 and performed CPR until EMS arrived. The initial 911 call was received at 6:02 pm.

Horn was last reported to have been in serious, but stable condition.
