(FORT WAYNE) - Dr. Cortney Schwartz, a practicing OB-GYN, has been suspended after having a sexual relationship with at least two patients and carrying on personal relationships with up to 12 other women.

According to the Medical Licensing Board of Indiana's summary suspension agreement, Schwartz reported to the Indiana Medical Licensing board director on September 28 that he had a sexual relationship with one patient and indicated another sexual relationship with one or two other patients since 2008-2009.

Then, on October 5, he admitted to having a personal relationship with up to as many as 12 women, either patients of his or patients of other practitioners in his practice group.

According to a spokesperson with the Indiana Medical Licensing Board, the Indiana Attorney General's office is investigating Schwartz's conduct. Once the investigation is complete, a hearing will be scheduled.

Schwartz has agreed to resign in the meantime. He voluntarily stopped practicing and has separated from his OB-GYN practice in Fort Wayne, where his address on file was 2518 E. Dupont Road, Women's Health Advantage. He also agreed to resign consulting privileges in hospitals in and around Fort Wayne.

On October 12, he underwent an evaluation in Kansas and is scheduled to enter into residence treatment on Nov. 30.

His medical license is suspended until at least January 28, 2016, when the Indiana Medical Licensing Board is scheduled to meet.

Women's Health Advantage issued the following statement:

"Women's Health Advantage no longer has any affiliation with Dr. Schwartz. For that reason, we are directing inquiries concerning his conduct to him."
