(UNDATED) - The Planned Parenthood clinic in Indianapolis has received a state reprimand for incomplete record keeping for abortions.

Audits from "terminated pregnancy reports" in 2007 indicated over 250 of them had missing information about the types of procedures performed and medications used.

Indiana Health Commissioner Dr. Judy Monroe threatened to go to the Marion County prosecutor if the clinic continued to file incomplete reports.

A Marion County grand jury is currently working on an unrelated investigation of the Indianapolis clinic involving an undercover video that exposed broken rules on reporting sexual abuse.

In a statement, Planned Parenthood says last summer it resubmitted the forms for abortion patient files requested by the state from as far back as 2006. It contends the state does not allow electronic submission of pregnancy termination reports, which it says would streamline the process and help avoid errors.
