(CLEVELAND, OH) - Travis Hafner’s walk-off grand slam capped a five-run bottom of the ninth for the Cleveland Indians, who beat the Toronto Blue Jays 5-4.

Travis Snider hit an RBI single for Toronto in the first and JP Arencibia and Rajai Davis added run-scoring singles in the fourth as the Jays took a 3-0 lead.

Jose Bautista made it 4-0 Toronto when he hit his major league-leading 29th home run of the season in the ninth off Cleveland's Tony Sipp.

In the bottom of the ninth, Blue Jays closer Frank Francisco gave up a single to Travis Buck, a double to Matt LaPorta and a walk to Jack Hannahan before being lifted in favor of Luis Perez.

Perez struck out Michael Brantley, but gave up an RBI single to Asdrubal Cabrera before surrendering the winning slam to Hafner.
