(BLOOMINGTON) - An Indiana Occupational Safety and Health Division report says workers severely burned in an August accident at the new Bloomington Wal-Mart store weren’t wearing proper equipment, and other safety procedures weren’t followed.
One of three injured men has since died. The other two remain hospitalized in fair condition.
The IOSHA report calls for a $15,000 fine against Muncie-based Electromation, the subcontractor that provided the workers.
