(FRENCH LICK) - Southern Indiana's French Lick Resort has been fined $14,000 by the Indiana Office of the Occupational Health and Safety Administration after failing to report an accident that led to the death of an employee.

The "News and Tribune" reports an employee at the golf course at the resort owned by Blue Sky Casino was initially injured in an accident in November of last year.

64-year-old Danny Walls died about ten days later from a blood clot related to the injury.

The accident and death were not reported for five months.
