(DAVIESS CO.) - Camp Illiana, a Christian youth camp, south of Washington in Daviess County, was cited by the Indiana Department of Labor for three serious safety violations after the tragic death of Camp Director David Bean.
According to IOSHA officials an investigation is generated anytime there is a fatality at a workplace or involves the death of an employee on the job.
60-year-old Bean was killed on July 12, 2014 by a falling tree limb. He and another employee 20-year-old Cody Chapman, of Vincennes, were removing a 75-foot tree, at the base of a hill located near the waterslide.
The men had completed cutting down the tree at its base and were attempting to pull it down with a Kubota tractor and a rope. The rope was tied to the front loader and attached to the tree.
Bean was standing near the base of the tree when the rope broke causing the tree to sway. That is when a large limb fell from the top of the tree and struck Bean in the head.
Camp staff called 911 and began first aid and CPR but were unable to save Bean.
The camp was cited for three violations for failing to provide and maintain a safe working environment while removing the tree that caused the fatality. Each violation provides for a fine of up to $2,100 or $6,300 total.
The camp has three options - they can request an informal conference and settlement; contest the findings; or correct the violations and pay the penalty as stated.
Camp Illiana has chosen the first option and should have a conference set within the next week.
