Tires Fall Off Semi, Causing Accident

(PAOLI) - Monday at approximately 7:40 a.m. the Indiana State Police responded to a three vehicle crash at US 150/SR 56 between Abydel Road and Castle Knoll Farm.

Preliminary investigation reveals that a green 2004 Mack trash truck driven by Douglas Kelm, 35, of Jasper was traveling East on US 150/SR 56 when the left rear wheels fell off traveled across the center line and hit a westbound white, 2007, International tractor-trailer, driven by Deneisa Hemmings, 53, of Paoli. After hitting the 2007 International the wheels traveled back across the centerline and hit a yellow, 2006, Hyundai, driven by Danielle Kendall, 27, of French Lick.

Kendall had minor injuries but refused treatment at the scene. Kelm and Hemmings were not injured.

The 2007 International had extensive front end damage and Kendall's Hyundai was a total loss.

The Indiana State Police Commercial Vehicle Enforcement Division conducted and post crash inspection on Kelm's vehicle and he was cited for equipment violations under the Federal Motor Carrier Safety Regulations for having loose lug nuts on the wheel and for the bolt holes on the wheel being elongated.
