(BEDFORD) - The Lawrence County Commissioners Tuesday morning passed an ordinance setting fees for service at the Lawrence County Health Department.
In nursing services:
Varicella vaccines $97
MMR (Mumps, Measles, Rubella) $57
Pneumonia $67
Document Completion fee $25
Notarized Immunization Record $5
Vital Record Services:
Paternity Affidavit $25
Notary Fee $2
Certified document fee, unless otherwise listed in fee ordinance $10
Environmental:
Homeowners self-install exam fee $50
Septic permit application fee $50
Additional review of permit or system requirements $25
Dye test $50
Lead case management $500
Pool permit fee per location annual fee year round operation $200
Pool permit fee per location seasonal operation $100
Food establishment annual (1-6 employees) $70
(7-13 employees) $100
(14-20 employees) $130
(21 plus employees) $160
Individual tattoo/body piercing artist license $50.
