Get immunizations before the start of the new school year

UNDATED – If your child will be starting kindergarten, 6th or 12th grade this fall they are likely due for booster immunizations prior to the start of the next school year.

Getting your child up to date on their vaccinations will help prevent against serious diseases.

Childhood immunizations are given at the Lawrence County Health Department by appointment only. Appointments are scheduled from 9 a.m. until 11:30 a.m. and 1:30 p.m. until 3:15 p.m. To schedule an appointment call 812-275-3234, ext. 2613 or 2615.

The Lawrence County Health Department is a member of the Children & Hoosiers Immunization Registry Program – C.H.I.R.P. 

Immunization records are required for all patients.

Copies of your immunization record may be obtained at the Lawrence County Health Department for a fee of $2. You will need to fill out an Immunization Record Request Form and bring identification.

The office is currently closed due to COVID-19 so call to request the records. No records are issued after 4 p.m.

Immunizations are offered for infants, children entering kindergarten, and students 11 through 18 years of age needing the State required vaccines. 

There is an $8 administration fee per shot. Insurance, Medicaid cards, and/or payment are required at the time of service.

In Monroe County, the Monroe County Public Health Department has appointments available every Monday in May from 8 a.m. until 6 p.m. You must have an appointment, so call 812-353-3244. Appointments will be held at the Monroe County Public Health Clinic at 333 East Miller Drive.
