Phyllis M. Frazier of Heltonville passed away Thursday evening at her home at the age of 70.
Born in 1937 in London, Kentucky to John and Grace Jones, Phyllis was a homemaker.
She is survived by her daughters Janica and Lisa, sons Rick, Mike and Randy, sisters Nell, Peggy, Judy, Kay and Betty, 6 grandchildren and several nieces, nephews and cousins.
She is preceded in death by her parents, husband Ralph, and brother Donald.
Services were held at 10:00am this morning at Ferguson-Lee Funeral Home with burial following at Hickory Grove Cemetery in Norman, Indiana.
